Leo ba247428a8 chore: bump 1.3.0 + CHANGELOG（vault 偵測 + Cowork skill） ...

承接上一個 feat commit：補上版號與更新紀錄，讓 update.sh 能通知用戶有新版。
- template/.claude/VERSION：1.2.0 → 1.3.0（新功能、向下相容）
- CHANGELOG：新增 1.3.0 一節
- .gitignore：排除 dev repo 自裝的 .claude/commands/（比照 wiki/hooks 的自用不推慣例）
