Leo 04ed722591 chore: gitignore .swarm/ + ruvector.db (Claude Code local artifacts) ...

之前 M1.2 telemetry commit 不小心把 cypher-executor/src/.swarm/ 跟 ruvector.db
commit 進去。這些是 Claude Code 自己的本地工具產物，不該入庫。

- 加 .gitignore 樣式：.swarm/ / ruvector.db / **/.swarm/ / **/ruvector.db
- 撤回上次 commit 進去的 5 個檔案
